An Advance Heath Care Directive specifies your wishes when you are unable to do so yourself. This includes what kinds of medical care you wish to be administered or withheld, your election on organ donation, naming a primary care physician, and specify a person to make medical decisions on your behalf. An Advance Heath Care Directive only take effect upon being incapacitated and no longer able to made medical care decisions for yourself. After created it can also be changed or revoked in your wishes change.
If you become incapacitated, a Durable Power of Attorney for Finances allow you to grant someone authority to handle your finances while you are unable to do so yourself. This can be important for both short and long term health issues.
A Final Arrangements Document specifies how you wish your remains to be treated upon death. This includes burial, cremation, organ donation, and name someone to oversee your wishes.
